How to Submit an Open Records Request

Online Submission

To submit an Open Records Request, please use our Request Portal. Creating an online account ensures that your request is submitted immediately and that all necessary information is provided for timely processing. You can also track the status of your request using your login credentials.

Non-Medical Open Records Requests

• Downloadable Form: If you prefer not to use the online portal, you can download a printable PDF Open Records Request form. Complete the form and submit it via:

• For Attorneys: Requests must be submitted through ChartSwap.com with the EMS Service listed as the provider.
• Non-Attorney Requests: Can be submitted via:
